DCBS Media ReleaseJune 7, 2001 State revokes Grants Pass insurance agents licenseSALEM – The Department of Consumer and Business Services (DCBS) has revoked the insurance agents license of a Grants Pass man, Brian R. Da Vault, for violating the Oregon Insurance Code. He also was fined $1,000. An investigation by the Insurance Division of DCBS found that Da Vault misrepresented information on an application for life insurance from a Milton-Freewater, Oregon, man. The man told Da Vault he had congestive heart failure and emphysema, but Da Vault misrepresented his health history on the application with Pioneer Life Insurance Co. Less than two years later, the man died of a stroke and his widow filed a claim for death benefits. Pioneer Life refused to pay, citing misrepresentation. After the Insurance Division intervened on the widows behalf, Pioneer Life reversed its decision and paid the claim. Da Vault is prohibited from selling or servicing insurance policies in Oregon. |
